Centriva Health is committed to protecting your privacy and handling your personal and health information in accordance with the Privacy Act 1988 (Commonwealth), the Australian Privacy Principles, and other applicable Australian healthcare privacy laws.

As part of providing healthcare services, we may collect personal information, medical history and other health-related details to support assessment, treatment, communication and continuity of care.By proceeding with a consultation, you consent to Centriva Health collecting, using and disclosing your information for purposes including:

  • Administration and management of healthcare services
  • Verification of identity and Medicare eligibility where applicable
  • Clinical assessment, diagnosis and treatment by healthcare practitioners
  • Communication with treating practitioners, specialists, pharmacies, pathology providers and other healthcare services involved in your care
  • Billing, payment processing and Medicare claiming where applicable
  • Quality assurance, staff education and service improvement activities
  • Compliance with legal, regulatory and professional obligations
  • Appointment reminders, follow-up communications and healthcare-related notifications
  • Secure storage and management of healthcare records
  • Use of secure digital systems and approved clinical documentation technologies to support healthcare delivery

Where permitted by law, consultations or communications may be monitored, reviewed or recorded for quality assurance, training, safety or compliance purposes.

Centriva Health may also contact you by phone, SMS or email regarding your appointments, healthcare services, account updates or important clinical information.

Medicare Benefit Assignment

Where bulk billing or Medicare rebates apply, you consent to assigning relevant Medicare benefits to Centriva Health or the treating healthcare practitioner for services provided. You acknowledge that Medicare claims and rebates are subject to eligibility requirements and current Medicare regulations.

Cancellation Policy

If you need to cancel or reschedule your appointment, Centriva Health requires at least 24 hours' notice prior to your scheduled consultation time.

Appointments cancelled with less than 24 hours' notice, missed appointments, or consultations where the patient does not attend (“Did Not Attend” or DNA) will incur the following fees:

  • Standard consultation: Full consultation fee charged
  • Extended consultation or specialised appointment: Up to 100% of the booked consultation fee may be charged
  • Repeated non-attendance or late cancellations: May result in restrictions on future bookings or prepayment requirements

Cancellation requests are only considered confirmed once you receive written confirmation from Centriva Health. These fees compensate for reserved clinical appointment time and the inability to offer that appointment to another patient at short notice. Centriva Health reserves the right to apply reasonable cancellation and non-attendance fees in accordance with Australian Consumer Law.

By signing or electronically accepting this consent form, you confirm that:

  • You have read and understood this consent information.
  • I have read and agree to the Terms of Service.
  • You consent to the collection, use and disclosure of your personal and health information as described above
  • You understand the limitations of telehealth consultations compared with in-person medical care
  • You consent to healthcare services being provided by appropriately qualified Australian-registered healthcare practitioners
  • You understand that prescriptions, referrals, certificates and treatment recommendations are issued at the practitioner’s independent clinical discretion
  • You consent to electronic communications relating to your healthcare and appointments
  • You understand that your information may be securely stored using approved healthcare technology systems and service providers
  • I consent to the use of AI-supported documentation
  • I understand that telehealth consultations do not provide emergency services. If I am experiencing an emergency, I understand that I can call 000 or Lifeline (13 11 14) for crisis and suicide support.
